Great twists.

My original thought was just how weird would it be to have an old character suddenly pop back to life. Instead of a month, I thought the delay might even be a year---just to make to less likely the PCs would hold up play, waiting for the resurrection.

Yeah, I saw what you're doing there, including the end-run around some frustrating Raise Dead RAW that a lot of us just do away with along with stuff like spell components and XP/Aging for crafting/wishes.
Before we get into it: I get why there's a terrible cost for these things and applaud all those who stick to those rules. My group doesn't, unless it's significant to the story or a player is crafting/wishing for something selfish/game-breaking. That's not an edition thing. We're munchkins. We've been doing it since 1.5e.

Looking at what I wrote, I suspect I was thinking of the item as an NPC item. The kind of thing you could drop in a mystery/horror adventure.

Amulet of Second Chances

This plain bronze medallion with a crude hour-glass relief grants to its wearer the ability to travel back in time 1 round and chose a different course of action. Only the wearer may change their stated action for that round, and all dice-rolls except the time-traveler's remain unaltered. The amulet disappears when used, and the user's body hair is immediately and permanently turned snow-white.

Veil of Shadows

When this spell is cast, the magic-user wraps himself in a swirling mass of darkness with both physical and psychological elements. Vision is obscured for 60' in all directions, making it impossible to identify the caster at the foci as well as targeting him difficult (+3 protection/+6 missiles). The veil can also be terrifying to behold, inducing irrational fears of a god-like presence. Creatures below 4 HD are compelled to either flee for one turn (50%) or are rooted to the spot, unable to act and likely (4 in 6) to drop whatever they are holding. More powerful opponents are allowed a saving throw to master their emotions.

The veil has far-reaching affects on a battlefield, reducing the morale of the opposing armies by one-third.

(This one is inspired by Sauron in Barad-dur, weaving shadows about himself as the Captains of the West gather outside of the Morannon.)

Lover's Quarrel
A red crossbow bolt with a broken hearted tip. This acts as a bolt+3, but if you strike a target who is romantically interested in you, or you have shared a bed with, they are instantly slain with no saving throw - their heart broken.

Some items based off my dogs -

Stick of Fetching

Throw the stick, and non-sentient monsters are compelled (provided they failed their save) to chase/retrieve it.

Silent Whistle

This whistle makes no discernable sound when blown except to any creatures within 20' with keen hearing; the high-pitched sound harms them for 2d6 sonic/thunder/whatever damage (they will prioritize attacking the blower, always).

Spellflinger

A ladle-like spoon device that can be charged with a ranged spell and used to fling that spell farther, doubling its casting range.
